Yellowhawk Tribal Health Center provides comprehensive healthcare services to eligible American Indians and Alaska Natives. New patients must complete an application process (patient registration) and provide required documents to establish their eligibility.

Tribal Verification
Enrollment card (with photo) from a Federally recognized Tribe, Certificate of Indian Blood, OR Proof of Native Descent.

Auricular acupuncture to address symptoms of withdrawal, depression, anxiety, and/or pain.
Substance Use Disorder
Assessment, outpatient treatment, inpatient referrals, Peer Recovery Mentoring, Connections App, and Oregon-licensed DUII Program.
Sober Transitional House

Horse Medicine Program
