Primary Care – Family Medicine

Our providers offer a wide range of care for the entire family from childhood through senior years, allowing you to get the care you need in one place for yourself and all of your loved ones. Providers at Yellowhawk have daily appointments for both management of chronic conditions as well as unexpected acute conditions requiring same day care.

Our care philosophy is based on the concepts of a patient-centered medical home. This is a comprehensive, team approach to care focused on prevention and helping our patients remain in good health, instead of simply responding to illness and injury. The medical home fosters trusting patient-provider relationships through improved access and consistency with your own primary care provider.

Our medical home team helps you navigate the health care system, including specialist and hospital care coordination and follow up.
